Fill out the form to register for the 25-26 school year. READ BELOW FIRST

• DO NOT ABBREVIATE on school names-use official school name- not nicknames such as SOUTH or ERHS. 
• Double check answers BEFORE SUBMITTING- WATCH FOR AUTOFILL It makes it difficult to contact coaches when they enter incorrect info- we had several registrations last year with errors in email or names which led to double registrations. If you are filling out for multiple people make sure you change names and roles on each form.
• Please have ALL coaches on your team use the SAME SCHOOL NAME and TEAM NAME when registering- it is very difficult for the membership director to put coaches together when they use different abbreviations for the school
• Payment can be online or by check.  Cost is $25 until December 1st. Cost will go up Dec 1st and the week of State competition. 
• TWO PAYMENT OPTIONS AVAILABLE

 1) pay $25 via Paypal  

 2) mail a check for $25 to MDTA Treasurer 1820 Highcrest Drive St. Charles, MO 63303  

NOTE: MAKE SURE TO INCLUDE NAME OF SCHOOL AND COACHES NAME IN WITH THE CHECK-  There is an option to print a receipt when done to mail with check.
